The Acute Stage of Ventilation: Supplying Ventilation to the Lungs in Patients With ARDS

چکیده
Suzanne M. Burns is a professor of nursing and an advanced practice nurse in the medical intensive care unit at the University of Virginia in Charlottesville. Mechanical ventilation is one of the most commonly used technologies in critical care. Despite the prevalence of ventilators, care planning is heavily influenced by anecdote and clinical preference. A vast array of ventilator modes and mode options exist, and claims of what works and what does not abound. Critical care nurses’ knowledge and understanding of mechanical ventilation are central to ensuring patients’ safe passage from the acute stage of ventilation to weaning. Of key importance is the use of evidence that may improve patients’ outcomes. To that end, in this article, I discuss the science related to mechanical ventilation in patients with acute respiratory distress syndrome (ARDS) and in patients who require weaning.

Mechanical ventilation in acute respiratory failure: recruitment and high positive end-expiratory pressure are necessary.
PURPOSE OF REVIEW To review as best the critical care clinicians can recruit the acute respiratory distress syndrome (ARDS) lungs and keep the lungs opened, assuring homogeneous ventilation, and to present the experimental and clinical results of these mechanical ventilation strategies, along with possible improvements in patient outcome based on selected published medical literature from 1972 ...

Although mechanical ventilation is a life-saving strategy in critically ill patients and an indispensable tool in patients under general anesthesia for surgery, it also acts as a double-edged sword. Indeed, ventilation is increasingly recognized as a potentially dangerous intrusion that has the potential to harm lungs, in a condition known as 'ventilator-induced lung injury' (VILI). So-called '...
متن کاملAcute Respiratory Distress Syndrome in a Patient With Suspected Influenza: A Case Report
Acute Respiratory Distress Syndrome (ARDS) is a clinical syndrome consisting of tachypnea, refractory hypoxemia, and diffuse opacities on chest radiographs after infection or trauma which ultimately leads to respiratory failure. The principles of treatment are based on patient care in ICU, mechanical ventilation and medical treatments. By using lower plateau, less tidal volume, higher positive ...
متن کاملSpontaneous breathing during mechanical ventilation in ARDS
The objective of mechanical ventilation used in the management of Acute Respiratory Distress Syndrome (ARDS) is to ensure adequate tissue oxygenation and alveolar ventilation while limiting the patients’ work of breathing and preventing further damage to the lungs. Although the “partial support” ventilation modes were initially developed to assist weaning or liberation from supported ventilatio...
متن کاملProtective mechanical ventilation, why use it?
BACKGROUND AND OBJECTIVES Mechanical ventilation (MV) strategies have been modified over the last decades with a tendency for increasingly lower tidal volumes (VT). However, in patients without acute lung injury (ALI) or acute respiratory distress syndrome (ARDS) the use of high VTs is still very common. Retrospective studies suggest that this practice can be related to mechanical ventilation-a...
